When a merchant is submitting a letter of error, the following will need to be included for consideration:

• Printed on the merchant’s company letterhead
• Accountholder’s name
• Account number (or) last 4 numbers of the account holder’s debit Mastercard®
• Transaction which caused the error
• Exact amount of transaction(s)
• Transaction dates(s)
• If the error caused fees to be assessed to the account, include that info in the letter
• An explanation from the merchant taking ownership of the error
